Objective

To ensure parent/guardian permission to publish student pictures, video and work on school-based websites.

 

Process

Lead Secretary:

1.       Distribute a Student Web Publishing Permission Form to the parents/guardians of each new student to the District.

2.       IF the completed form is returned, code Pentamation as follows:

 

¨       If the parents/guardian object to publishing the child’s picture/video  ONLY on a Portland Public Schools supported web page – code Pentamation work(no picture).

¨       If the parents/guardian object to publishing the child’s work ONLY on a Portland Public Schools supported web page – code Pentamation

picture (no work).

¨       If the parents/guardian object to publishing BOTH the child’s picture/video and work on a Portland Public Schools supported web page, code Pentamation none.

¨       If the parents/guardian do NOT object to publishing both the child’s picture/video and work on a Portland Public Schools supported web page, code Pentamation both (work & picture).

 

3.       File the completed Student Web Publishing Permission Form in the students’ “cum” files.

4.       IF the completed form is NOT returned, Pentamation will default to none.

5.       Print 1 copy of school’s Student Web Publishing Permission Report and retain in the school office for reference.

6.       Update the report as new student enroll.

 

Teachers:

1.       Collect Student Web Publishing Permission Forms from students.

2.       Return forms to Lead Secretary.

3.       IF student picture, video, or work is requested for publishing on a school-based website, check the Student Web Publishing Permission Report in the school office to ensure parent/legal guardian permission.

4.       IF permission has not been granted (and given that Pentamation defaults to none), you may want to contact parents/legal guardian for permission.  A Student Web Publishing Permission Form must be completed.

 

NOTE:  This process must be done only once – at enrollment.
